Team Registration
Gather your friends, family members, co-workers, members of your faith group or book club and form a team!
Teams compete for the Golden Sneaker Award in 3 categories: Team that raises the Most Money, Team with the Most Spirit, and Team with the Most Members.
Each team will receive an 8 x 10 photo of your group taken at the event as well as special team-tags to be worn during the event. Feel free to wear hats, buttons, or anything else to destinguish your team.
Forming a Team is as easy as 1-2-3!
- Recruit a minimum of three team members. Teams must have a captain and at least two members. A $60 minimum team donation ($20/person) is requested.
- Decide on a team name.
- Register your team with the form below or call 773-465-6011 ext. 114 to register by phone.
